Medical and Release Form

We require a Medical and Release Form to be completed prior to participation in all courses. Because of the ever-changing nature of medical and other necessary information, a new form must be completed for each course you or your child attends.

Other Medical Forms

Our youth and adult backcountry courses (such as our adult and teen expeditions) and all youth overnight camps require that an additional medical form be completed in order to participate. This form is called the Physician Examination Form. In most cases it needs to be turned in to us a minimum of one month prior to the course start date. Please call the office if you have questions about this form: 425-788-1301.

Scholarship Forms

If you would like to apply for one of our need-based scholarships for our courses, you have the choice to either complete an On-line Scholarship Application or download the Scholarship Application in pdf format, and mail it in. (See our scholarships page for more information).
